Teesside Hospice in Teesside is seeking a therapy assistant to join the Wellbeing and inpatient unit (IPU) team. The role involves delivering treatment plan activities, supporting patients at the inpatient unit, Wellbeing centre or in their homes, and working under supervision.

You will contribute to care pathways for people with long-term conditions, assess needs, plan visits, document on SystmOne and liaise with the MDT. NVQ 3 or BTEC in care and experience in adult health are required.

Qualifications

• NVQ level 3 in care or BTEC in health and social care.
• GCSE passes in maths and English (A-C).
• Experience in adult health or social care settings.

Responsabilités

• Contribute to care pathways for patients with long-term conditions and palliative rehabilitation needs.
• Carry out clinical duties and procedures within own competencies and scope.
• Apply competency-based treatment techniques under supervision, including exercise programmes.
• Assess patients' needs and adjust care within own skills and competence.
• Order and supply therapeutic equipment and instruct on use.
• Communicate health needs and treatment plans to patients and carers; deliver education programs such as BIC.
• Build therapeutic relationships to motivate patients and boost confidence.
• Plan and prioritise visits; document on SystmOne.
• Collaborate with MDT and liaise with community teams to coordinate care.
• Report changes in patients' condition and share information with the team.
• Follow safeguarding policies; respect dignity and involve patients, carers and family in shared decision making.
• Travel independently across areas covered to deliver care.
