Job Summary

An exciting opportunity has arisen for a Band 7 Macmillan Palliative Care Clinical Nurse Specialist to join our dynamic team at Imperial College Healthcare.

As a Palliative Care Clinical Nurse Specialist, you will provide expert palliative care nursing advice to support and facilitate high-quality, individualized care to patients and their relatives/carers, in collaboration with the multidisciplinary team, in line with local and national targets.

The successful candidate will manage their own caseload of patients, working as part of the multidisciplinary Specialist Palliative Care Team. They will maintain effective communication with patients, families, carers, and other healthcare professionals to ensure seamless service delivery.

As part of the team, you will undertake comprehensive palliative care assessments to identify and diagnose patients' physical, social, psychological, cultural, and spiritual needs, facilitating quality nursing care and contributing to clinical care decisions.

Main Duties
  • Ensure appropriate referral and discussion with team members to optimize patient care.
  • Assess patients, develop, implement, and evaluate care plans according to Trust's evidence-based procedures.
  • Maintain open, transparent communication channels within the multidisciplinary team.
  • Act as a clinical expert for palliative and end-of-life care.
  • Ensure staffing aligns with Trust policies.
  • Manage budgets and resources effectively.
  • Promote a culture of learning and quality improvement.
  • Represent palliative and end-of-life care across the Trust.
