Job summary

Chelsea and Westminster Hospital NHS Foundation Trust in London is seeking a BAND 6 Paediatric Nurse to join the PATCH team, providing assessment and treatment for children at home with support from NWL PATCH Team and the Virtual Wards Programme.

The post involves independent clinical assessment of acutely unwell children, home visits, and coordination with hospital teams; a driving licence is required; there is potential weekend work at multiple sites and a six‑month probation period.

Qualifications

• Part 1 Registered Nurse – Child or equivalent.
• Level 3 Safeguarding.
• PILS certificate.
• Advanced/Enhanced Assessment Course.
• Evidence of further study.
• Experience working in a Hospital @ Home team.
• Experience working in the Emergency Department.
• Community nursing experience.
• UK Driving Licence or equivalent.
• Excellent communication and interpersonal skills.

Responsibilities

• Ensure high standards of patient care at all times and uphold professional conduct.
• Ensure local plans support clinical quality and external accreditation (CQC).
• Embed a culture of quality improvement and safe care.
• Provide leadership and support to the nursing team and monitor patient progress.
• Store, check and administer drugs according to Medicines policy.
• Occasionally work off-site and conduct home visits as required.
• Maintain accurate nursing documentation.
