Job summary

Central London Community Healthcare NHS Trust is seeking an experienced Clinical Nurse Specialist for the Brent Community Paediatric Team. You will provide specialist nursing assessment, care coordination and ongoing support from referral to long-term care for children and young people aged 0–19 with special needs.

You'll work within a multidisciplinary team, develop personalised health plans, and contribute to service development with clinical leadership.

Qualifications

• Experience with children and young people in multidisciplinary settings.
• Specialist Community Practitioner qualification desirable.
• Ability to develop personalised care plans and coordinate pathways.

Responsibilities

• Triage referrals and provide families with information and support to access services.
• Undertake comprehensive assessments of children and young people with complex needs.
• Coordinate specialist care with follow-up via clinics, calls, and home visits as appropriate.
• Contribute to EHCNAs and produce health reports for EHCPs.
• Run nurse-led clinics and review care during medication changes or MDT reviews.
