Job summary

Change Grow Live in Leeds is seeking a dedicated outreach worker to support individuals with complex needs across 7 days. The role involves night shelter duties, collaboration with partner services, and detailed record-keeping of interactions and support plans.

The position is full-time, fixed-term for 12 months, offering development opportunities, wellbeing resources, and a supportive team environment.

Qualifications

• Experience of engaging and providing meaningful support to individuals with complex support needs.
• Experience of communicating effectively with people requiring support.
• Experience of working in a hostel/supported housing environment.
• Knowledge of Safeguarding Procedures and their application.
• Experience of working with people in crisis.

Responsibilities

• Going on outreach shifts; these include early mornings, nights, and weekends. The service covers 7 days.
• Engaging with rough sleepers and people involved in street based anti-social behaviour.
• Working in the night shelter/winter accommodation overnight in shifts over a 7 day period.
• Working with partner agencies to support service users to access accommodation, physical and mental health services, substance use treatment services, benefits etc.
• Accurately recording interactions with the people we work with, other professionals, and related support plans.
