Job summary

Devon Partnership NHS Trust is seeking motivated, compassionate support workers, OT technical instructors and assistant psychologists to join the new Learning Disability and Autism Outreach Team across Devon. The Band 4 post involves collaboration with LDA Outreach colleagues and The Brook inpatient unit to deliver complex, person-centred care.

You should have clinical experience with learning disabilities and/or autism, be able to work autonomously within a MDT, and manage a demanding workload

Qualifications

• Experience with learning disabilities and/or autistic people.
• Knowledge of safeguarding, MHA and MCA.
• Ability to work autonomously within defined care pathways and in MDT.
• Experience with risk assessment, formulation and complex care planning.

Responsibilities

• Triaging, assessing, planning, formulation and delivering complex care packages.
• Liaise with The Brook inpatient unit and other partner agencies and families.
• Support discharge planning and post-discharge follow-up within Devon.
