Pay Rate: $18.50/Hour
Job Summary:
The Outreach Case Manager would provide a holistic approach to case management services and providing outreach programs to participants who are returning from incarceration and/or at risk of becoming incarcerated. This position will require effective interaction and coordination with participants, staff members, volunteers, and other agency representatives to facilitate client success.
Essential Functions:
Engage in regular outreach to returning citizens with screenings and intakes.
Ability to interact with participants that are in situations such as low-income, post-addiction, and incarceration.
Knowledge of intake and interview techniques with a holistic approach.
Provide case management with ability to assess situations and work independently to develop successful action plans with goals and objectives addressing the individuals current needs while preserving the participants dignity.
Maintain contact with participant and ensure follow through with the goals, objections, and expectations created.
Scheduling meeting times with participant by being creative and flexible to their needs. Traveling is required.
Coordinate with local community services to provide available programs for participants.
Develop a relationship with local law enforcement, detention centers, and community corrections.
Ability to work as part of a case management team and build effective working relationships with co-workers and other agencies/organizations.
Maintain accurate case records, reports, and statistical data.
Excellent interpersonal, verbal, and written communication skills.
Working knowledge of standard office procedures and report preparation.
Proven ability to maintain confidentiality within and outside of agency.
Minimum Requirements:
High School Diploma or equivalent required.
At least one-year of experience providing case management services.
Working knowledge with local Human Services.
Ability to pass pre-hire screenings such as background or drug screenings.
Valid drivers license and access to a vehicle on a continual basis.
