Job summary

Sussex Partnership NHS Foundation Trust is seeking experienced mental health professionals to join the Rapid Response service across Sussex. Roles involve on-scene and telephone crisis interventions, person-centred assessments and developing onward care plans.

Practitioners will work with a community support worker across a 24/7 rota. Responsibilities include on-scene assessments or telephone-based care, recording outcomes and coordinating with emergency services to prevent unnecessary

Qualifications

• Experience delivering mental health crisis interventions.
• Ability to carry out person-centred assessments.
• Able to coordinate onward care plans and referrals.

Responsibilities

• Carry out person-centred assessments during crisis.
• Develop onward care plans addressing presenting needs.
• Provide hear-and-treat interventions by telephone when appropriate.
• Attend scenes to conduct face-to-face assessments when required.
• Complete written records of referrals, interventions and outcomes.
• Advise on alternative pathways to avoid unnecessary conveyance.
• Collaborate with emergency services and other agencies to achieve best outcomes.
