About the Role

This position is within the Mental Health Reablement Team, a short-term service aimed at preventing and reducing social care needs while promoting independence and recovery in the community. The team includes social workers, occupational therapists, and assistant practitioners. The goal is to deliver targeted, brief interventions to enhance wellbeing and independence, helping service users avoid long-term social care support.

The Occupational Therapist will:

  • Manage a caseload and deliver Reablement interventions to prevent or reduce needs related to mental ill health.
  • Provide specialist OT assessments and prescribe equipment when appropriate.
  • Support team members with expert assessments and interventions.

Experience
