This role is within the Mental Health Reablement Team, a short-term service aimed at preventing and reducing social care needs while promoting independence and recovery. The team includes social workers, occupational therapists, and assistant practitioners. The goal is to deliver targeted, brief interventions to enhance wellbeing and independence, helping service users avoid long-term social care support.

The role requires expanding knowledge in assessment and support planning, specialist OT assessments, safeguarding, resource management, healthcare needs, mental capacity, and complex family issues.
Understanding of reablement principles, wellbeing, strengths-based, and rehabilitative practices is essential. Additional knowledge includes equal opportunities, anti-discriminatory and anti-oppressive practices. Qualifications such as AMHP, AMCP, or BIA are desirable or willingness to train.
