Job overview

An exciting opportunity has arisen for an enthusiastic therapist with acute NHS experience to join our Integrated Discharge Team (IDT) working in A&E and other in-patient assessment areas.

You may already be a senior therapist looking for a new challenge or a Band 5 ready to make the move into a specialist area. As a band 6 you will be expected to perform to a high clinical standard, supervise staff and carry out service development. You will be supported by an experienced team leader.

The team works closely with acute & community services, providing a comprehensive assessment of patients to avoid admission. We are an integrated team supported by a social care practitioner, specialist nurses and consultant geriatricians. The service operates over seven days, between 8am and 6pm and the successful candidate will be rostered regular days of work within this.

The successful applicant will be able to demonstrate a flexible approach to the assessment of older adults in crisis, acknowledging the broad aspect of medical and social factors influencing frailty. Main duties of the job

The post holder assists in the development, administration and provision of a specialist Occupational Therapy service; carries a varied caseload of recently discharged patients. Managing complex assessment, with predominantly physical conditions and with regard to the physical, emotional, psychosocial, spiritual and social aspects of their care.

Detailed job description and main responsibilities

Directly responsible for the management of own caseload of patients referred for IDT assessment, treatment and management within the multi-disciplinary team/community.

Assists in the management of the specialist team providing, developing and planning an Occupational Therapy service to patients for this client group.

Person specification
Qualifications
  • iploma/degree in OT/PT by recognised awarding body e.g. Royal College of Occupational therapists/CSP.
  • Must satisfy the criteria for full membership of the Health and Care Professions Council (HCPC)
  • Further qualifications/Study
  • An approved supervisor for OT/PT degree students
Knowledge
  • Knowledge of the role of Occupational Therapy for people with a wide range of physical dysfunction.
  • Able to assess for and fit basic equipment to facilitate safe discharge
  • Previous experience relating to specialist area
  • Experience of working in an acute hospital setting
  • Knowledge of relevant NHS and SS legislation
  • Able to assess for and fit complex equipment and teach patient/carer in the safe use of these
  • Experience with community/mental health conditions
