• Help people maximise independence and access the support they need to live well.
• Enable people to achieve outcomes that matter, promoting independence, wellbeing and inclusion.
• Work alongside teams including Adult Early Help, Reablement, Technology Enabled Care, Sensory Services and Occupational Therapy.
