Sanctuary Personnel is seeking an experienced Occupational Therapist to join the First Contact Service Team in Sutton, London. This full-time, ongoing post offers an hourly rate of up to £37.85, with a blend of office-based and remote working days to support flexible service delivery.

You will screen referrals, conduct person-centred assessments and work within a multidisciplinary team to promote independence and safety for residents, including adaptations and signposting to the right services.

• Screen and prioritise complex referrals to meet immediate needs.
• Conduct person-centred, strength-based assessments to inform plans.
• Guide Enabling Independence Officers and Trusted Assessors.
• Collaborate within a multidisciplinary duty team for community support.
• Recommend equipment/adaptations to promote independence.

Responsabilidades

• Screen and prioritise complex referrals from the Contact Centre.
• Conduct assessments and formulate effective support plans.
• Provide guidance to officers and assessors to ensure service quality.
• Work with the multidisciplinary team to deliver community support.
• Signpost to services and assist with adaptations and housing needs.
• Demonstrate safe use of equipment to clients and families.
• Liaise with Housing, Social Care and partner agencies.
