Job summary

Sirona care & health – England’s largest provider of Adult’s and Children’s Community Services – invites applications for a fixed-term post to support the ICSS team in Bristol, North Somerset or South Gloucestershire. You’ll work under supervision of stroke specialist clinicians, delivering home and community based stroke assessment and rehabilitation with a focus on promoting independence.

Responsibilities

• Provide specialist assessment and rehabilitation for people with stroke in their homes and local community
• Work with the MDT to support transfer of care from hospital to home enabling flow from the hospital setting
• Collaborate with other professions to provide holistic care to patients and their families
• Provide informal supervision for rehabilitation support workers and students
• Set up and monitor treatment plans for personal care, meal preparation and therapy programmes for support staff to follow
• Collaborate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan
• Facilitate appropriate referrals to other services or investigations based on your assessment to ensure holistic patient care
• Assess and arrange the provision of equipment and/or adaptations from appropriate agencies ensuring this is provided in good working order and fitted correctly; reporting any issues appropriately

Job description

This is a fixed term post to support the ICSS team through times of high demand. ICSS is a supportive and friendly MDT providing assessment and rehabilitation for people in their homes and local community. Supervision would be provided by stroke specialist clinicians and you would have access to inservice training and advice from clinicians in the team. This is a great opportunity to develop your skills in stroke rehabilitation and occupational therapy

• To work closely with other professions to provide holistic care to patients and their families
• To provide informal supervision for rehabilitation support workers and students

• Collaborate and negotiate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan, which will maximise quality of life and independence.
• Facilitate appropriate referrals to other services or investigations based on your assessment to ensure holistic patient care
• To assess and arrange the provision of equipment and/or adaptations from appropriate agencies ensuring this is provided in good working order and fitted correctly; reporting any issues appropriately
