Job summary

Leeds Community Healthcare NHS Trust is hiring a Band 6 Senior Occupational Therapist to work in the Active Recovery Service or Neighbourhood Therapy Teams. You will assess adults after illness, injury, hospital admission or functional decline, and build and deliver personalised rehabilitation plans to support recovery and independence.

You will provide evidence-based interventions to improve function, mobility and quality of life, manage a varied caseload across community or intermediate care,

• Work with multidisciplinary teams to plan and coordinate care
• Contribute to integrated care pathways in community and intermediate care services
• Supervise students and support staff
• Take part in service development and quality improvement initiatives
• Engage in ongoing professional development and clinical supervision

Job description

A Band 6 Senior Occupational Therapist post with Leeds Community Healthcare NHS Trust, placed in either the Active Recovery Service or the Neighbourhood Therapy Teams. The work centres on assessing adults after illness, injury, hospital admission or functional decline, then building and delivering personalised rehabilitation plans. Active Recovery is short-term, goal-focused intermediate care run with Leeds City Council, while the Neighbourhood Teams provide community rehabilitation focused on falls, mobility and preventing admission.
