Occupational Therapist - Community Access Team

Contract: 3-Month Initial Contract with potential for extension

Location: Chester (Hybrid)

Role Overview

I am currently recruiting an Occupational Therapist on behalf of my client, who is seeking to join their Community Access Team. The team operates on a multidisciplinary model, integrating Occupational Therapy and Social Work, with a focus on an OT-first approach to enhance independence and delay or prevent the need for further services.

Responsibilities

• Address complex housing situations and manage related tasks.
• Screen for Disabled Facilities Grants (DFG).
• Handle initial enquiries related to moving and handling.
• Utilise reablement services and issue equipment.
• Perform minor adaptation work and leverage technology-enabled care.
• Maintain awareness of safeguarding principles and collaborate with the multidisciplinary team to ensure safety and effective risk management.

Requirements

• Valid Occupational Therapy qualification and HCPC registration.
• Office presence required, with flexibility for intermittent remote work.
• Possession of a valid driving license and access to a vehicle during working hours.
• Ability to conduct a significant portion of work over the telephone.
