Job summary

Pennine Care NHS FT is seeking a Registered Nurse Associate to join the Tameside GM SMI Team. You will help run and develop this arm of the Physical Health & Treatment Team, focusing on patients with Severe Mental Illness and supporting access to Primary Care Physical Health Screens across primary and secondary care settings.

The role emphasizes continuity of care, effective communication, and the promotion of parity of esteem between mental and physical health.

Qualifications

• Registered Nurse Associate with Foundation Degree.
• GCSEs Grade C/4-9 in Maths & English Language or equivalent.
• Experience of working with individuals with mental health needs or in distress.

Responsibilities

• Liaising with and supporting GP practices in reviewing the SMI register.
• Working alongside Primary Care partners to support the service.
• Provide support to enable patients to engage with health needs and arrange initial appointments.
• Support patients at risk, escalate concerns appropriately.
• Promote understanding of the link between mental and physical health.
