Job summary

Whittington Health NHS Trust is seeking a Registered Nursing Associate to join the Haringey MACCT, delivering proactive, holistic care to adults 18+ with frailty and complex needs. You will work closely with GPs, therapists, social care and pharmacists to coordinate care.

The role includes frailty assessments, care planning, MDT presentation and escalation of risks. It requires NMC registration and clinical supervision within the MACCT framework.

Qualifications

• Experience of working under supervision as part of a MDT.
• Ability to work effectively as part of MDT.

Responsibilities

• Undertake follow up assessments for patients previously reviewed by senior clinicians.
• Complete actions identified during Comprehensive Geriatric Assessment, initiating referrals within competencies.
• Undertake frailty assessments following frailty induction and supervisor sign-off.
• Present patients at MDT meetings and act as a care coordinator when appropriate.
• Escalate changes, emerging risks, or concerns promptly.
• Manage an allocated caseload independently within competence.
• Reassess patients holistically and elevate concerns via internal pathways.
• Collaborate with GPs, Physiotherapists, Social Workers, Pharmacists, Occupational Therapists and others.
• Act in the best interests of patients to promote integrated care.
• Monitor health needs and adapt care with colleagues, families and carers.
