Nuneaton & Bedworth Primary Care Network is recruiting two Frailty Nurses to support two Integrated Neighborhood Teams. The role involves working with multidisciplinary colleagues to identify, assess and manage frailty, reducing hospital admissions and improving continuity of care for elderly patients across practices, care homes and community services.

You will work across Manor Court Surgery and Redroofs Surgery, contributing to INT goals and frailty programs while maintaining strong

Qualifikationen

• Significant post-registration nursing experience with frail/elderly patients in community settings.
• Experience presenting at multidisciplinary team (MDT) meetings.
• Current NMC registration and driving licence to travel between sites.

Aufgaben

• Identify patients with moderate to severe frailty and perform CGA.
• Develop and regularly review personalised care plans with patients and carers.
• Lead weekly care home rounds and liaise with GPs and care home staff.
• Coordinate MDT input and ensure joined-up, patient-centred care.
