LPN/RN Hall Nurse

Job Description

Department: Nursing

Reports to: Director of Nursing, Assistant Director of Nursing

Reporting to this position: None

Job Classification: Staff Nurse

Position Purpose: Provides nursing care to residents assigned.

Required Qualifications
  • Must be a graduate of an approved LPN/RN program.
  • Training in rehabilitative and restorative nursing practices is preferred.
  • Minimum training in a hospital, skilled nursing care facility, or related medical setting.
  • Current CPR Certification.
  • Ability to work independently within the scope of practice for LPN, with flexibility, personal integrity, and effective teamwork skills.

Must meet state licensure requirements and have no disciplinary actions against professional license.

Major Duties and Responsibilities
  • Participate in planning and implementing unit activities as assigned by the DON and/or RN Supervisor.
  • Ensure policies, procedures, safety rules, and regulations are followed by department personnel, residents, visitors, etc., intervening when violations occur.
  • Provide accurate information to personnel, residents, visitors, family members, etc.
  • Protect residents' rights, including fair treatment, privacy, and confidentiality.
  • Maintain adequate stock levels of medications, supplies, and equipment; requisition as needed.
  • Notify appropriate medical and legal personnel in case of critical incidents.
  • Work with facility consultants and implement recommended changes.
  • Handle death procedures as per policy, including body preparation and funeral arrangements.
  • Report and chart resident conditions, incidents, and care interventions accurately.
  • Assist in medication administration, treatment, and resident care activities.
  • Orient and evaluate unlicensed personnel performance.
  • Review physician’s orders, transfer information, and ensure documentation is current.
  • Participate in resident admission, discharge, and transfer processes.
  • Follow infection control protocols and safety measures.
Additional Tasks
  • Treat residents with dignity and respect, promoting residents' rights.
  • Adhere to all policies, procedures, and regulations, including safety and confidentiality.
  • Report noncompliance, abuse, or safety concerns promptly.
  • Participate in safety drills, training, and quality improvement activities.
  • Use proper body mechanics and safety procedures during care delivery.
Physical and Personal Skills
  • Effective communication, teamwork, and decision-making skills.
  • Physical ability to perform resident care activities, including lifting and mobility tasks.
  • Ability to work beyond normal hours, including weekends and holidays, when necessary.
  • Basic computer skills and familiarity with electronic medical records.
Compliance and Performance

Adherence to all facility policies and participation in performance evaluations are mandatory. Violations may lead to disciplinary action.
