Responsibilities

• Provides direct care to patients in accordance with individualized care plans.
• Assists patients with activities of daily living (bathing, oral care, dressing, grooming, toileting).
• Assist in transportation and transfer of patients, ensuring comfort and safety during movement.
• Monitor and record vital signs, intake and output, reporting abnormalities to nursing staff.
• Collect specimens as directed and maintain specimen procedures.
• Perform accuchecks as directed and document results.
• Maintain accurate and prompt documentation.
• Assist with patient hygiene and comfort, including turning and repositioning to prevent pressure ulcers.
• Support feeding and hydration, adhering to dietary restrictions and assisting with mealtime activities.
• Collaborate with the healthcare team to ensure continuity of care and positive outcomes.
• Complete hourly rounds and respond to call lights and patient requests promptly.
• Provide emotional support and companionship to patients and families with empathy.
• Maintain clean, organized patient rooms, change bed linens, stock supplies, and prepare rooms for admissions and transfers.
• Adhere to infection control protocols and hospital policies.
• Participate in ongoing education to enhance patient care skills.
