Job summary

Cleveland LMC Ltd. is seeking an experienced Nurse Practitioner to join Hirsel Medical Centre in Middlesbrough, delivering autonomous clinical care across acute and chronic conditions within a multidisciplinary team.

You will assess, diagnose, treat, prescribe, and refer appropriately, contributing to improved access to primary care services and high-quality patient care.

Qualifications

• Registered Nurse with current registration with the relevant professional body.
• Qualified Nurse Practitioner with advanced clinical assessment skills.
• Experience working in primary care or general practice.
• Knowledge of UK primary care systems and pathways.
• Experience using System One EPR.

Responsibilities

• Conduct comprehensive patient assessments and clinical examinations.
• Diagnose and manage acute and long-term health conditions.
• Independently prescribe medications within your scope of practice.
• Order, interpret, and act upon diagnostic tests and investigations.
• Provide health promotion, disease prevention, and lifestyle advice.
• Refer patients to secondary care and specialist services when required.
• Maintain accurate and timely patient records.
• Work collaboratively with GPs, practice nurses, pharmacists, and other healthcare professionals.
• Participate in clinical governance, audit, and quality improvement activities.
