Job summary

The Nurse Practitioner role at Hirsel Medical Centre in North Ormesby Health Village offers autonomous clinical care for patients with acute and long-term conditions. You will assess, diagnose, prescribe within your scope and refer where needed, working closely with two GP partners, a practice nurse and a healthcare assistant.

You will join a small, friendly team, contribute to clinical governance and quality improvement, maintain accurate records and support health promotion and disease

Qualifications

• Qualified Nurse Practitioner with advanced clinical assessment skills.
• Independent Prescriber qualification.
• Current registration with the NMC (Nurse & Midwifery Council).
• Experience in primary care or general practice.

Responsibilities

• Conduct comprehensive patient assessments and clinical examinations.
• Diagnose and manage acute and long-term health conditions.
• Independently prescribe medications within scope of practice.
• Order, interpret and act upon diagnostic tests and investigations.
• Provide health promotion, disease prevention and lifestyle advice.
• Refer patients to secondary care and specialist services when required.
• Maintain accurate and timely patient records.
• Work collaboratively with GPs, practice nurses, pharmacists and other healthcare professionals.
• Participate in clinical governance, audit and quality improvement activities.

Job description

The Nurse Practitioner post sits within Hirsel Medical Centre, a small GP practice in North Ormesby Health Village with around 4,000 patients. The role centres on autonomous clinical care for people with acute and long-term conditions, including assessing, diagnosing, prescribing and referring. The practice has two GP partners, a practice nurse and a healthcare assistant, so the post works closely with a small team. Applicants need NMC registration, an independent prescriber qualification and primary care experience.
