Job summary

Qualifications

Job Summary

Are you an experienced Senior Nurse looking for an opportunity to utilise your proven leadership skills alongside patient clinical assessment and acute care delivery? If so, this unique role is for you. The Acute Care at Home service is a partnership between the University Hospital of North Midlands (UHNM), University Hospital of Derby and Burton (UHDB), Royal Wolverhampton Trust (RWT) and Midlands Partnership Foundation Trust (MPFT). The successful applicants will join an integrated, collaboratively focused team, employing proactive clinical practice and knowledge, toprovide clinical assessment, monitoring and support, as part of our urgent carepathways. The post holder is required to hold a non-medical/supplementary prescribing qualification. However, applications will also be considered from clinically experienced applicants with the relevant skillset currently undertaking the course or willing to undertake on appointment within the first 12 months.

Take an active role to ensure that evidence-based practice is maintained to deliver the Virtual ward agenda supporting earlier discharge from hospital and admission avoidance. Hold delegated managerial responsibilities under the supervision of the Matrons and provide ongoing support to junior members of the team, participating in induction and training programmes.
