Job Summary

Are you an experienced and motivated clinician looking to make a real difference by delivering high-quality care closer to home?

We are seeking a Band 6 Clinical Practitioner to join our innovative Urgent Community Response (UCR) and Virtual Wards service. This exciting role offers the opportunity to work across integrated community pathways that support admission avoidance, early supported discharge and hospital-level care delivered in patients' own homes.

You will undertake comprehensive clinical assessments, support clinical decision-making, coordinate care across multidisciplinary teams and contribute to the delivery of responsive, patient-centred care. Working within a dynamic and supportive team, you will help people receive the right care, at the right time, in the place they call home.

We are looking for a registered healthcare professional with excellent assessment and communication skills, experience of managing complex patients, and a passion for integrated community care. Experience in urgent care, frailty, community services or virtual care models would be advantageous.

Main Duties, Tasks & Skills Required

The UCR/VW team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission within their own home within 2 hours of referral to the service. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors

The post holder will work in the Urgent community response team and Virtual Wards to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.

The post holder will ensure access to Urgent Health Assessments, provide and monitor short term treatment programmes at home and ensure safe discharge or referral to long term health and social care interventions. Utilising clinical expertise, clinical judgement, discretion and decision making in the development of individualised treatment.

To work in partnership with primary, secondary and other health and social care providers, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.

To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.

The post holder will be expected to work within a flexible working pattern over a 7 day week 24/7 in the urgent community response team as required.

Qualifications

Essential

Evidence of professional and clinical knowledge acquired through training to degree/diploma level leading to Professional registration on the appropriate part of the HCPC / NMC register i.e. (Adult / Child / Learning Disability or Mental Health)
Evidence of undergraduate education for newly qualified nurses / or post graduate continuing professional development for registered nurses who have post registration experience

Desirable

Health care management / leadership qualification
Mentorship / assessor training qualification.

Experience

Essential

Substantial post registration experience working in a complex health care setting / Acute NHS care setting supervising / mentoring junior staff
Evidence of professional /clinical knowledge in area supplemented by specialist clinical and or managerial training and CPD
Can demonstrate understanding / experience / willingness to be a practice-based supervisor / assessor as detailed in the Nursing & Midwifery Council ( NMC ) standards for student supervision and assessment
Professional portfolio which demonstrates continuing professional development, evidence of ability to maintain professional registration
Relevant and recent clinical experience within the speciality /acute care setting, including the supervision /education of junior staff
Experience of mentoring undergraduate students
Evidence of ability to lead /coordinate a shift /area of department
Experience in learning from incidents /complaints and supporting changes in practice.
