Job summary

Highwoods Surgery is seeking a nurse practitioner/advanced nurse practitioner to deliver high-quality clinical care to patients across primary care. The post is part-time (24 hours per week) with independent prescribing responsibilities and GP support within a collaborative team.

The ideal candidate will manage minor illness, acute disease presentations, and chronic conditions common in the community, including asthma and hypertension. Training and professional development are encouraged.

Qualifications

  • NMC registration is required.
  • Independent prescribing qualification is required.
  • Experience in community/primary care preferred.
  • Ability to work independently with GP support.

Responsibilities

  • Diagnose and treat patients with acute medical conditions, minor illness and mental health problems, including a mix of face-to-face and telephone consultations.
  • Ensure clinical practice is safe and effective and work within boundaries of competence.
  • Prescribe within competencies to patients following national and local formulary.
  • Initiate and arrange investigations and referrals to other primary and secondary care team members as clinically indicated.
  • Participate in administrative and professional responsibilities of the practice team including clinical audit.
  • Ensure the clinical record is completed and maintained with accurate details.
  • Participate in education and training of students and induction of staff where appropriate.
  • Actively participate in clinical and whole-team meetings to promote information sharing.

About the Role

Highwoods Surgery is proactively seeking a nurse practitioner/advanced nurse practitioner, to provide high quality clinical care to our patients, seeing a variety of patients across the range of primary care. The role is offered on a part-time basis, of 24 hours per week. The ideal candidate would be able to consult with patients and manage minor illness and acute disease presentations, as well as be knowledgeable about acute presentations of chronic diseases that would be commonly dealt with in the community such as asthma, COPD, hypertension and cardiac conditions. They would be able to work independently but would have collaborative support from the GPs and wider team. A culture of discussion and shared learning is encouraged.

  • Diagnosing and treating patients with acute medical conditions, minor illness and mental health problems, including a mix of face to face and telephone consultations. Patients presenting via triage, prebooked appointments or care home.
  • Ensuring clinical practice is safe and effective and working within boundaries of competence.
  • Prescribe within competencies to our patients keeping in line with the national and local formulary.
  • Initiating and arranging appropriate investigations and referrals to other members of the primary and secondary health care teams as clinically indicated.
  • Participating in the administrative and professional responsibilities of the practice team including clinical audit.
  • Ensuring the clinical record is completed and maintained with accurate and contemporaneous details recorded.
  • Participating in the education and training of students of all disciplines and the induction of all members of the practice staff where appropriate.
  • Actively participate in clinical and whole-team meetings, as this promotes information sharing, supports effective communication and helps maintain positive and professional working relationships within the workplace.

About the Candidate

To be considered for this position, you will need the following qualifications, skills and experience:

  • Professional registration with the NMC
  • Strong interpersonal and communication skills and be able to liaise effectively with the clinical and administrative staff within the practice and partnership
  • An ability to demonstrate innovation and awareness of future opportunities and challenges would be desirable to help meet the current and future challenges of primary care, and to help us promote a rewarding environment for staff as well as ensuring excellent patient care
  • An independent prescribing qualification
  • C hildhood, adult and travel immunisation preferable, but not essential
