Qualifications

• Relevant professional qualification such as Registered Nurse (NMC), Nursing Associate, Healthcare Assistant, or equivalent care certificate; community/home care experience is beneficial.
• Strong clinical and caregiving skills including personal care, medication support, monitoring vital signs, and responding to changes in health and wellbeing.
• Excellent communication and interpersonal skills to build trust with people receiving care, families, and colleagues, including clear record-keeping and reporting.

Responsibilities

• Provide person-centred care and support to individuals in their homes and local community.
• Assist with personal care, medication administration, and mobility support.
• Monitor health and wellbeing in line with care plans and document care accurately.
• Communicate changes in condition to clinical leads and collaborate with multidisciplinary teams to ensure coordinated support.
