Job summary

Primary Integrated Community Services Ltd is seeking a compassionate Nurse Associate to join our Primary Care team in a GP practice environment. You will support management of long-term conditions, deliver preventative care and perform a range of clinical interventions, working with GPs, Practice Nurses and other health professionals.

You will benefit from strong supervision, ongoing professional development and pathways to progression toward becoming an RGN.

Qualifications

  • Registered Nurse Associate on the NMC register with a Nursing Associate Foundation Degree.
  • Excellent communication and teamwork skills.
  • Ability to deliver safe clinical care and carry out patient assessments.

Responsibilities

  • Deliver health checks, chronic disease monitoring, venepuncture, ECGs, immunisations and wound care.
  • Promote health and wellbeing and support adherence to screening programmes.
  • Maintain accurate clinical records and support care planning within a multidisciplinary team.
  • Provide patient assessments and participate in service improvements.

Job description

We are looking for a compassionate and motivated Nurse Associate to join our Primary Care team within a GP practice environment. This is an exciting opportunity to work with a diverse patient population, supporting the management of long-term conditions, delivering preventative healthcare and carrying out a range of clinical interventions. You will work closely with GPs, Practice Nurses, Healthcare Assistants and other health and social care professionals, helping to provide high-quality, patient-centred care. The role offers excellent support and supervision, with a strong commitment to ongoing professional development and progression, including support for Nursing Associates who aspire to become Registered General Nurses (RGNs).,

  • Deliver a range of clinical interventions including health checks, chronic disease monitoring, venepuncture, ECGs, immunisations, wound care and patient assessments to support high-quality patient care.
  • Promote health and wellbeing by supporting patients to manage long-term conditions, encouraging participation in screening programmes and providing advice to improve health outcomes.
  • Work collaboratively within a multidisciplinary team, maintaining accurate clinical records, supporting care planning, contributing to service improvements and ensuring safe, effective and compassionate care.

    Registered Nurse Associate on the NMC register with a Nursing Associate Foundation Degree qualification.

  • Ability to deliver safe clinical care, including patient assessments, long-term condition monitoring and health promotion activities within scope of practice.
  • Excellent communication, organisational and teamwork skills, with the ability to use clinical systems, prioritise workload and maintain accurate patient records.
