Job summary

The Confederation, Hillingdon CIC invites nurses to join our dynamic operations team across multiple primary care services in Hillingdon, London. You will work with clinical leads and an experienced management team, with travel between sites as required to meet service needs.

You must be NMC registered with a UK pin and hold relevant practice nurse training. You will perform cervical smears, immunisations, BP checks and chronic disease reviews, contributing to QOF and patient care improvements

Qualifications

• Registered with the NMC and eligible to practice in the UK.
• Experience in NHS settings and general public care.
• Strong IT and data recording skills.

Responsibilities

• Carry out cervical smear tests, dressings, HRT reviews, immunisations, BP checks, asthma reviews and diabetic reviews.
• Assess health needs and determine if further input is needed.
• Screen patients for risk factors and early signs of illness.
• Record data accurately for QOF and PCN requirements.
• Develop care plans in line with current protocols and educate patients.
• Maintain governance, CPD and updated clinical protocols.

Job Summary:

We have a new and exciting opportunity to appoint nurses to join our dynamic and friendly operations team. The post-holder will have the opportunity to work across different primary care services. You will be dealing with a wide range of patient health needs across the local area. You will be supported in your role through the clinical leads across the services you are working in as well as an experienced and friendly management and administration team. This post may require you to travel between sites within the borough of Hillingdon as and when required you must be registered with the Nursing and Midwifery Council (NMC) and hold pin and a certificate of completion of training.

Primary Responsibilities

• In accordance with the services needs the post-holder will make him/her-self available to undertake a variety of duties including cervical smear tests, post-operative dressings, HRT reviews, Pill Checks, Immunisations, BP checks, asthma checks and diabetic reviews and spirometry if trained.
• Assessing the health needs of patients with chronic health needs and using your own judgment to assess whether further input is needed.
• Screening patients for disease risk factors and early signs of illness.
• Contributing to QOF requirements and recording data appropriately.
• Contributing to PCN requirements and recording data appropriately.
• In consultation with patients and in line with current practice disease management protocols, developing care plans for health related illnesses.
• Screening patients for health education.
• Admitting and discharging patients to and from the caseload and referring to other care providers as appropriate.
• Recording clear and contemporaneous consultation notes to agreed standards.
• Collecting data for audit purposes and contributing to the practice audit programme as required.
• Take responsibility for some aspects of the clinical work within the team, for example: coordinating aspects of governance, QOF, enhanced services, CPD and keeping clinical protocols up to date.
• In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a practice nurse working within primary care, as set by Royal College Nursing and the Nursing and Midwifery Council and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups and all other regulatory and standard setting organisations.

• Experience of working within the NHS, with general public & team working
• Well developed IT skills and an enthusiasm for new applications and software

• Able to obtain and analyse complex technical information

• Experience working within primary care
• Certificates for Practice Nurse Training and cervical Smears Training with relevant registration numbers.
