The role involves working closely with therapists, fracture clinics, patients, families, and partner services to deliver high-quality, person-centred care. Responsibilities include coordinating patient pathways, progressing exercise programmes within competency, providing equipment and mobility aids, maintaining accurate records, and supporting smooth patient flow through the service. The post holder will contribute to multidisciplinary working, pathway development, and continuous service improvement, while ensuring safe and effective care delivery.
Car ownership and the ability to drive is a requirement of this role as this is a community post.
