Job summary

Main duties of the job

We are looking for people who:

• Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required
• Working as part of a 24/7 service and multidisciplinary team

• To work as an autonomous practitioner with the integrated multi professional team
• To be accountable for assessing interpreting, planning, implementing and evaluating treatment to patients within professional guidelines To keep the patient at the centre of care, ensuring patient and carer participation in decision making
• Provide support and education of peers, new staff, non registered staff, and students
• To provide high standards of nursing intervention within a patients own home, this includes lone working , with access to specialist and advanced practitioners when required

Person specification

Education / Qualifications

• Ability to travel independently around the Cambridgeshire and Peterborough area meeting required timescales. A full driving licence and access to a vehicle is essential.
