Job summary

EBPC is seeking a Practice Nurse to join our extended service in Bracknell. You will deliver high-quality nursing services to the entitled patient population and work with the multidisciplinary team to ensure comprehensive care.

Responsibilities include health promotion, chronic disease management, and women/men’s health services within your scope. You will support policy review and maintain high standards of care in a collaborative environment.

Qualifications

  • Nursing qualification and current registration with the NMC.
  • Postgraduate qualification in Primary Care or equivalent preferred.
  • Evidence of immunisation training and competence in the last two years.

Responsibilities

  • Identify and support patients at risk of long-term conditions.
  • Deliver routine nursing care based on clinical evidence.
  • Provide wound care and order pathology tests as required.
  • Run chronic disease clinics and refer to secondary/specialist care when needed.
  • Maintain accurate clinical records and ensure safeguarding compliance.

Role Summary

As a practice nurse in our extended service in Bracknell, your primary responsibility is to provide high-quality nursing services to the entitled patient population. In addition, you will work collaboratively with the multidisciplinary practice team to ensure that patients receive comprehensive and coordinated care.

Your role will involve delivering care within your scope of practice, which includes several clinical areas such as health promotion, chronic disease management, health prevention, and well‑women and well‑man clinics. Additionally, you will support the management team in reviewing clinical policy and procedures to maintain the highest standards of care.

Main duties of the job

Primary Responsibilities:

  • Identify, manage, and support patients at risk of developing long‑term conditions, preventing adverse effects on the patient's health.
  • Provide routine nursing care to patients as required by clinical‑based evidence, NICE and the NSF.
  • Provide wound care to patients.
  • Request pathology services as necessary.
  • Provide chronic disease clinics, deliver patient care as necessary, referring patients to secondary / specialist care as required.
  • Maintain accurate clinical records.
  • Ensure Stomed codes are used effectively.
  • Maintain chronic disease registers.
  • Prioritise health issues and intervene appropriately.
  • Recognise, assess, and refer patients presenting with mental health needs.
  • Support the clinical team with all safeguarding matters following local and national policies.
  • Deliver opportunistic health promotion where appropriate.
  • Maintaining smears results in logs.
  • Manage emergency drug kit.
Health and Safety

Infection Prevention and Control

You will be expected to cooperate with colleagues, contractors, and other healthcare providers involved in patient care directly or indirectly so far as necessary to enable EBPC to meet its obligations under the Health and Social Care Act 2008.
