Job summary

You Matter in Liverpool is seeking a Night Support Worker to provide empathetic, person-centered care for residents recently discharged from hospital.

You will help create a safe environment, carry out risk assessments, support daily living needs, and collaborate with health services to promote recovery and social inclusion.

This is a night position that supports residents and contributes to their successful reintegration into the community.

Responsibilities

• Welcoming residents in line with service policies and supporting them during their time in the service.
• Ensure basic needs are met including accommodation, facilities, healthy food, advice and guidance.
• Advocate for social inclusion and help residents access health and financial support.
• Collaborate with residents to address health needs and liaise with primary/secondary health teams.
• Hold activities in service and encourage participation and involvement from residents.
• Provide immediate support and crisis intervention as needed.
• Raise health, safety and maintenance issues promptly.
• Participate in team meetings and reflective practice, with open feedback for improvement.
• Show flexibility and responsiveness to changing service needs and uphold SHREK values.
• Promote a supportive and inclusive culture within the service.

As a Night Support Worker at You Matter, you will naturally be an empathetic and caring person wanting to provide unconditional support for those experiencing mental health issues.

Your role as a Night Support Assistant will be to create a safe environment that empowers residents to make positive changes in their lives.

Nightingale House is a resettlement service that provides 25 beds of accommodation to adults that have recently been discharged from hospital following a mental health admission.

We work in partnership with MerseyCare and our service helps people to build life skills, settle back into the community and find suitable long-term accommodation.

What your week may look like

• Welcoming new residents in line with the service's policies and procedures, carry out risk assessments and support people during their time in service.
• Ensuring the basic needs of residents are met including good quality accommodation and facilities, healthy food, advice and guidance.
• Advocate for social inclusion of the people you support ensuring they can access health and financial support where required.
• Work alongside the people in our services to address their physical and mental health issues, including liaison with primary and secondary health teams.
• Holding activities in service and encourage participation and involvement from the residents.
• Carrying out the immediate support needs of residents as appropriate, providing crisis intervention where necessary and working with services.
• Ensuring all health and safety and maintenance issues are raised in a timely manner
• Participating in team meetings, supervision and reflective practice encouraging open, honest, and transparent feedback where continuous improvement is encouraged.
• Being flexible and responsive to the changing needs of the service, ensuring our SHREK values runs through all the work that you do.
• Promoting a supportive and inclusive culture within the service, valuing people and their diversity.
