Job summary

Avery Healthcare Group is seeking a warm, enthusiastic Team Leader for Acer Court Care Home in Nuthall, Nottinghamshire. You will support the home management team to deliver high-quality care in a safe, open and inclusive environment for residents, visitors and staff.

Applicants should have NVQ 3 in care, at least two years in a senior role with residents, and strong communication and leadership skills. ADBS disclosure checks and UK work eligibility are required.

Qualifications

• NVQ 3 or equivalent in care home settings.
• Two years’ experience in a senior role with residents.
• Strong communication and leadership abilities.

Responsibilities

• Lead the person-centred care planning process with monthly evaluations.
• Perform care plan audits and develop action plans.
• Manage discharge processes, including documentation and medications.
• Keep next of kin informed where appropriate.
• Monitor residents' wellbeing and update care records as needs change.
• Maintain safe working practices and manual handling procedures.
