Purpose of the Post:

To work as part of a multi-disciplinary team and assist professional members of the Contact Cares service to provide integrated, multidisciplinary programmes of reablement aimed at promoting independence and helping to restore/maintain the service user’s ability to remain within the community.

Duties and Responsibilities:

• Remain alert and aware during the night shift, undertaking regular and discreet checks on service users, ensuring all checks and incidents are appropriately recorded in accordance with Departmental and CQC guidelines.
• Perform regular checks on exit doors, windows, waste bins, and electrical appliances, adhering to health and safety requirements and being mindful of fire, trip, and security risks.
• Attend to the personal care needs of service users, including washing, bathing, changing, and toileting.
• Assist the District Nursing Services in maintaining appliances such as catheter and colostomy bags.
• Make beds, handle soiled linen, ensure service users have adequate clean clothing, and notify the Duty Manager for replacements or repairs.

Knowledge:

• Understanding of rehabilitation processes enabling older people to return home and live independently.
• Knowledge of Health and Safety issues.

Skills and Abilities:

• Positive interpersonal skills with clients, carers, and colleagues.
• Effective verbal and written communication skills.
• Ability to work independently and as part of a team.

Experience:

• Caring for elderly and disabled individuals in formal or informal settings.
