Job summary

West London NHS Trust invites applications for a Band 5 Community Nurse to work night shifts in the Hounslow area, based at Clayponds Hospital. You will deliver essential nursing care to housebound patients, collaborating with the Community Nursing Team and external services to prevent hospital visits.

The role requires current nursing registration, a driving license, and strong communication and teamwork abilities. Shifts run 19:00–07:00, seven days a week, offering Twilight and Overnight slots.

Qualifications

• Registered General Nurse with UK registration.
• Full UK driving license and access to a car.
• Strong communication skills and ability to work in a team.

Responsibilities

• Provide high quality nursing care to housebound patients with complex needs.
• Assist Community Nursing Team Manager in planning, implementation and evaluation of care.
• Coordinate with out-of-hours services and other health professionals to meet patient needs.
