Care UK is hiring a Care Team Leader to supervise shifts, manage resident care plans, and coordinate with families and healthcare professionals. You will administer medication, perform risk assessments, and work closely with care assistants to promote independence and high-quality care.

The role also involves collaborating with leadership to develop services and maintain a positive, inclusive team environment.

Formación

• Strong leadership and supervisory experience.
• Willingness to train on administering medication.
• Ability to develop care plans and coordinate with families and professionals.

Responsabilidades

• Lead and supervise your shift to ensure smooth operations.
• Case manage residents and liaise with families, healthcare professionals and stakeholders to deliver best care plans.
• Administer and record medication as per care plan.
• Carry out risk assessments to safeguard residents.
• Work with care assistants to support daily needs and promote activity and independence.
• Collaborate with the leadership team to develop services and improve care delivery.
• Provide a positive, inclusive and professional approach at all times.
