Connect2Hampshire is recruiting a Neurodiversity/Family Support Practitioner to help families and schools access holistic, strengths-based support for neurodivergent learners. You will work closely with families, schools and partner agencies to coordinate services and promote inclusive practice.

The role emphasizes remote delivery and requires experience across teaching, mental health or related fields, with a neuro-affirmative approach to improving outcomes for children and families.

Qualifications

  • Experience in teaching, mental health, therapies, social care, family support, or related field.
  • Experience supporting children and families with neurodivergence and SEND.
  • Knowledge of neurodevelopmental conditions including Autism, ADHD, DCD and DLD.
  • Excellent communication and partnership-working skills.
  • Positive, proactive and neuro-affirmative approach.

  • Act as a key link between families, schools and partner agencies, helping to coordinate support and build positive, cooperative relationships.
  • Support schools to strengthen inclusive practice, improve attendance and reduce barriers to learning for neurodivergent pupils.
  • Contribute to multidisciplinary working, including support planning and participation in MDT reviews as this aspect of the service develops.
  • This role is predominantly remote and good knowledge / experience using various platforms and systems is essential.

Job description

You'll be part of an innovative service driving a shift from a diagnosis-led approach to a needs-led model. Working closely with families, schools and partner organisations, you'll help ensure children and young people receive timely, holistic support based on their strengths and individual needs, while promoting inclusive practices that enable neurodivergent learners to thrive.

What you'll do

What we're looking for
  • A relevant experience in teaching, mental health, therapies, social care, family support, psychology or a related field.
  • Experience supporting children, young people and families, ideally with a range of needs across the spectrum of neurodivergence, SEND, and family support contexts.
  • Knowledge of neurodevelopmental conditions, including Autism, ADHD, Developmental Coordination Disorder (DCD) and Developmental Language Disorder (DLD).
  • Excellent communication and partnership-working skills, with the ability to build trusted relationships and provide clear, practical guidance.
  • A positive, proactive and neuro-affirmative approach, with a commitment to improving outcomes for children, young people and families.
