Job summary

Horwich Primary Care Network in Bolton is seeking a pharmacist to join a general practice team, delivering clinical medicines expertise and patient-facing care. • Support day-to-day prescribing functions and medicines reconciliation on transfer of care
• Collaborate with community and hospital pharmacy to optimise medicines use and safety
