Job summary

The Pharmacist Network is recruiting an IP Clinical Pharmacist for a partner practice in Salford. The post is fully or part-time, onsite, focusing on delivering pharmaceutical care and medication optimisation within general practice.

You will work with pharmacists and technicians to resolve queries, perform reviews and improve patient access to medicines. Ideal candidates will have GP/general practice experience, GPhC registration, independent prescribing, and familiarity with EMIS or SystemOne.

Qualifications

  • Minimum 12 months experience working within general practice/primary care.
  • GPhC registration as a Pharmacist/Clinical Pharmacist is required.
  • Independent prescribing qualification is desirable and beneficial.

Responsibilities

  • Provide pharmaceutical care and advice to patients within a general practice setting.
  • Resolve medication-related queries and support clinicians.
  • Conduct polypharmacy reviews (including SMRs) and face-to-face med reviews.
  • Support medicines optimisation and contribute to quality improvement in practice.
  • Educate patients on medications and disease prevention.
  • Lead with safe, appropriate and efficient use of medicines.

Our senior clinical team support the training, development and workflow management of Pharmacists and Pharmacy Technicians within primary care and PCN settings. We currently have an exciting opportunity for an IP Clinical Pharmacist, to work full time or part-time for our partner practice within Salford, this opportunity is available for highly motivated and focused Clinical Pharmacist, who will be responsible for the smooth running of day-to-day activities within general practice, while playing a vital role in providing pharmaceutical care and advice to patients focusing on best practice and adhering to safety guidelines. As an onsite Clinical Pharmacist, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians. You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvement of access to healthcare.

  • Ensure appropriate and efficient use of medication
  • Educate patients on medication and disease prevention
  • An understanding of QOF, CQC and local incentive scheme workstream
  • Create and follow clinical pharmacy programme according to practice policy
    A minimum of 12 months experience of working within general practice/primary care
  • A relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registration
  • Independent prescribing qualification
  • Experience with EMIS or System 1
  • Completed the CPPE Primary Care pathway or close to completion
  • Able to commute to onsite location / live within close proximity of practice
  • Excellent communication and interpersonal skills, Experience with EMIS and SystemOne
  • CPPE Pathway Completed
