Job summary

Central London Community Healthcare NHS Foundation Trust is seeking a Registered Nurse or Allied Health Professional to join its community services in Potters Bar. The role focuses on delivering patient-centered care within a multidisciplinary team in the community setting.

You will work with service users with complex health needs, contribute to care planning, and support students and learners, while ensuring accurate documentation and adherence to NHS guidelines.

Education/Qualifications

Essential criteria

• Registered Nurse (RN) or AHP
• NMC Registration / HCPC Registration

Desirable criteria

• Evidence of post-registration education and training relevant to role
• Non medical Prescriber

Experience

Essential criteria

• Recent clinical nursing or AHP experience negotiating and liaising within a multidisciplinary team
• Working with people with complex health needs
• Supporting students and other learners e.g. HCAs teaching others in the practice environment
• Experience of undertaking comprehensive assessment by utilising evidence based care.
• Previous experience in the Nursing or AHP process, care planning and fluent in record keeping
• Experience of providing holistic care that involves the multidisciplinary team.
• Post registration experience in a NHS community environment

Desirable criteria

• Experience of training other students.
• Participation in audit; research projects; service development
• Experience in completion of advanced care plans and treatment escalation care planning

Skills and Knowledge
