Job summary

Mid Hampshire Healthcare is seeking a compassionate Registered Nurse to join our expanding clinical team in a varied Multi-Service Nurse role across Hampshire and Isle of Wight sites. Mid Hampshire Healthcare is looking for a compassionate, motivated and adaptable Registered Nurse to join our friendly and expanding clinical team in a varied Multi-Service Nurse role.

This is a unique opportunity to work across a range of community healthcare services throughout Hampshire, including Enhanced Access, Women's Health, long-term condition clinics, community and outreach services, and our Mobile Health Bus. With such a varied portfolio, no two days are the same.

Working as an autonomous practitioner within your scope of practice, you will undertake patient assessments, deliver evidence-based nursing care and clinical interventions, support preventative healthcare and health promotion, and work alongside GPs and other healthcare professionals to provide high-quality, patient-centred care.

Main duties of the job

As a Multi-Service Registered Nurse, you will work across a variety of MHH services and clinical settings, providing safe, effective and patient-centred nursing care according to service need.

Your working week may include supporting Enhanced Access clinics, long-term condition management, Women's Health services, community and outreach clinics, or working from our Mobile Health Bus.

You will assess patients, plan and deliver appropriate nursing care and undertake clinical procedures within your individual competencies and scope of practice. You will also support preventative healthcare, health promotion and patient education, recognising and escalating clinical or safeguarding concerns where appropriate.

You will work both autonomously and alongside GPs, Advanced Clinical Practitioners, Healthcare Assistants and other members of the multidisciplinary team, maintaining accurate clinical records and ensuring effective communication and continuity of care.

Flexibility is central to the role. You will work across different services and locations and have the opportunity to support new and developing services as MHH's clinical portfolio continues to evolve.

The Multi-Service Registered Nurse will deliver high-quality, patient-centred nursing care across Mid Hampshire Healthcare's portfolio of community healthcare services.

Working as an autonomous practitioner within their scope of practice, the postholder will undertake holistic patient assessments, provide evidence-based nursing care, deliver health promotion and preventative interventions, and contribute to the ongoing development of innovative community healthcare services.

The postholder will work collaboratively within multidisciplinary teams, ensuring patients receive safe, effective and compassionate care whilst maintaining the highest standards of clinical practice, professionalism and patient safety.

The role will involve working across multiple MHH services and locations, including GP practices, community clinics, mobile health services and other commissioned locations, depending on operational requirements.

The successful candidate will demonstrate excellent clinical judgement, strong organisational skills, flexibility and a commitment to delivering outstanding patient care.

Key Tasks and Responsibilities

The postholder may be required to:

Deliver safe, effective and evidence-based nursing care across a variety of MHH services. Undertake comprehensive patient assessments and formulate appropriate care plans. Deliver treatment and clinical interventions within NMC standards and organisational policies. Support the delivery of long-term condition management and preventative healthcare. Deliver health promotion and patient education. Undertake clinical procedures appropriate to their competence and service requirements. Work autonomously whilst recognising professional limitations and escalating concerns appropriately. Maintain accurate clinical records using EMIS and associated digital systems. Participate in multidisciplinary team meetings and collaborative working. Work across clinic, community and outreach settings. Support the introduction and development of new healthcare services. Contribute to clinical governance, audit, quality improvement and patient safety initiatives. Maintain infection prevention and control standards. Fulfil safeguarding responsibilities in accordance with local and national guidance. Maintain equipment, medicines and clinical stock appropriately. Participate in mandatory training, appraisal, clinical supervision and Continuing Professional Development. Support Healthcare Assistants, students and newly appointed staff where appropriate. Work flexibly across MHH services according to operational need.

Training and Professional Development

Training requirements will be monitored through annual appraisal and professional revalidation.

The postholder will:

Maintain NMC registration and comply with the NMC Code. Maintain Continuing Professional Development (CPD). Develop and maintain a Personal Development Plan. Attend mandatory training. Participate in clinical supervision. Undertake additional training required to support new and developing services.

Essential Qualifications

Registered Nurse (Adult). Current NMC registration. Evidence of ongoing CPD. Full UK driving licence.

Essential Experience

Minimum two years' post-registration experience. Experience within Primary Care, Community Nursing or NHS services. Good IT skills. Experience of electronic patient records.
