Job summary

Look Ahead Care, Support and Housing Ltd seeks a kind, compassionate and resilient Multi-Disciplinary Support Worker for its Homelessness Social Care Service in Westminster. The role offers £31,593 per annum, 40 hours per week, and a permanent contract with generous benefits.

You'll join a team dedicated to transforming lives, with learning and development opportunities and a supportive culture. Weekly hours are Monday–Friday with a Sunday rotation and some on-call coverage.

Qualifications

  • Experience in social care or homelessness services.
  • Ability to cope positively with challenging and diverse behaviour.
  • Excellent communication and interpersonal skills.
  • Willingness to work flexible shifts as described.

Responsibilities

  • Build supportive relationships with customers and partners.
  • Perform holistic assessments with relevant referrals.
  • Lead regular key-work sessions to achieve plans.
  • Develop support plans based on ongoing assessments.
  • Assess and manage risks with a positive risk-taking approach.
  • Support access to services and empower self-management of medications.

Edward Alsop Court provides support and accommodation for up to 79 men over 35 with mental health conditions complex needs.

These needs may be around physical and mental health, alcohol and drug use, housing, benefits and finances, life skills development, and much more.

The working pattern for this role consist of: Monday to Friday 9am-5pm, one Sunday per month (Friday off on this week), and occasional 12pm-8pm shifts.

What you'll do:

  • Building supportive, trusting relationships with customers and external partners, creating a positive atmosphere
  • Carrying out holistic assessments of new customers which incorporate relevant statutory referral information
  • Conducting regular key work sessions that are innovative and engaging in order to achieve Support plan goals
  • Create support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customer to progress in their recovery
  • Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
  • Provide support and access to relevant services to enable customers to continue their recovery and empower customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making This is not an exhaustive list of all the duties and responsibilities that may be required from time to time and is subject to change in accordance with the needs of Look Ahead About you: Enjoys social interaction and the company of others, joins in local activities to encourage customer involvement Exudes a warm, friendly presence Prefers working as part of a group or team Is fundamentally calm and resilient, does not let emotion adversely affect them or obscure their judgement Has a practical and logical mind and is naturally well-organised Thrives on change and enjoys dynamic diverse environments Is confident with high levels of self-esteem Is respectful, articulate and sensitive in style of communication Is motivated towards excellence and improvement of personal performance with a can do attitude Ability to cope positively with challenging and diverse behaviour What you'll bring: Essential Facilitates case conferences for all customers Supports a caseload of customers Builds and develop strong professional partnerships with external providers Provides necessary shift cover in line with rota requirements About us: Look Ahead is a leading, not-for-profit care and support provider in London and the South East.
