Job summary

Change Grow Live is seeking a part-time Dual Diagnosis Engagement Worker in London to support individuals with co-existing mental health and substance use needs. The role involves working across community and inpatient settings, including Acute Psychiatric Wards and Community Mental Health Services, using assertive in-reach approaches.

You will assess, plan and coordinate personalised support with a multidisciplinary team, ensuring safeguarding and recovery-focused pathways.

Qualifications

• Experience in supporting people with complex mental health and substance use challenges.
• Ability to work across community and inpatient settings.
• Trauma-informed, person-centred approach.

Responsibilities

• Conduct comprehensive assessments and screening.
• Offer pharmacological and psychosocial interventions.
• Provide support reducing harm and involve families and carers.
• Make onward referrals to appropriate services.
