Job summary

The role involves delivering person-centred care, developing skills, and creating a safe, nurturing environment with a focus on empowerment and inclusion. You will travel across urban and rural areas using your own vehicle, follow care plans, document progress, and liaise with families, therapists, and colleagues.

Qualifications

• Proven experience working with adults and children with learning disabilities or autism.
• Valid driving license with clean record; driving is essential.
• Ability to create engaging activities tailored to individual needs.
• Experience developing and following detailed care plans for children with special needs.
• Excellent communication with children, families, and team members.
• Proficiency in using IT systems for documentation and reporting.

Responsibilities

• Provide engaging and supportive care to people with learning disabilities and autism, and life-limiting conditions when applicable.
• Develop and implement individualized care plans in collaboration with families, therapists, and professionals.
• Maintain clear communication with family and team members regarding progress and changes in care routines.
• Use IT tools to document daily activities, update care plans, and record progress accurately.
• Monitor behavior and well-being, responding swiftly to emergencies or behavioral challenges.
• Ensure compliance with health and safety regulations during transportation and in the care environment.
