Job summary

PennineCare is seeking a Mental Health Practitioner to join the Young Onset and Rarer Dementia team in Stockport. The role involves community-based assessment and care planning for clients under 65 and their carers, with a strong emphasis on risk and cognitive functioning.

The successful candidate will work within a multidisciplinary team, provide input on care packages, and uphold professional standards and safeguarding policies.

Qualifications

• RMN or appropriate registered AHP
• Short courses and experience to post graduate diploma level
• Evidence of continuing professional development
• Trained to degree level
• Demonstrable experience in mental health
• An accredited mentor for nursing students

Responsibilities

• Carry out thorough mental health assessments including risk and cognitive functioning
• Make clinical decisions for individual patients in community settings
• Assess and manage co-morbidity of physical and mental health and medication effects
• Provide advice on activities of daily living for service users and carers
• Communicate clearly with service users and carers about risk and care plans
• Plan and implement client-centered interventions with MDT input
• Act as care coordinator or care manager as required
• Adhere to safeguarding policies and mandatory training
• Maintain confidentiality and high professional standards
• Contribute to audits, data collection and research opportunities
