Job summary

Pennine Care NHS Foundation Trust is seeking a Senior Mental Health Practitioner for the Heywood, Middleton and Rochdale Neighbourhood Mental Health Team. You will work in the community with individuals to navigate mental health services and related activities, promoting health, wellbeing and quality of life.

The role includes initial assessments, person-centered planning, risk management and signposting to interagency resources, with brief interventions offered where appropriate.

Qualifications

• Applicants must possess RMN / RNMH / Social Worker / OT or equivalent.
• Completion of Preparation for Mentorship / Practice Teacher/ is desirable.
• Significant post-registration experience required.

Responsibilities

• Provide biopsychosocial assessments and collaborative next-step planning.
• Screen referrals and devise risk management plans for service users as part of duty.
• Offer brief interventions (up to 12 weeks) including coping strategies, problem solving and motivational work.
