Midlands Partnership University NHS Foundation Trust is recruiting a Mental Health Practitioner to join the Crisis Resolution Home Team based at Hall Court, Telford. The role involves delivering specialist assessment and care planning, and working within a multi-disciplinary team to provide 24/7 crisis support.

As part of the Urgent Care Pathway, you will evaluate treatment plans, undertake home treatment where appropriate, triage referrals, and coordinate team activities.

Our Crisis Resolution Home Team is looking to recruit a Mental Health Practitioner.

The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week on a varying rostered shift pattern. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.

We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists Social Workers, Administrators and Business Support Staff. As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.

In the role you will deliver a high-quality clinical service to patients and their families.

You will deliver therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.

The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week on a varying rostered shift pattern.

Full time hours are 3 x 11 hour and 15 minute shifts with an additional 7 hour 30 minute shift per week.

33.75 hours post will work 3 x 11 hour and 15 minute shifts per week.
