Sussex Partnership NHS Foundation Trust is recruiting 0.8 WTE Mental Health Practitioners based at Mill View Hospital, Hove, with home visits and on-site assessments. The role focuses on community-based mental health care, collaboration with carers and multidisciplinary teams, and delivering high-quality, compassionate services.
Ideal candidates are registered nurses, OTs or social workers with CPD, mentorship training, and extensive experience in risk assessment and community settings.
The service is looking to appoint candidates who are highly motivated, enthusiastic, creative and resourceful, and who have a passion for working with people and for developing good working relationships with carers. You will have the opportunity to forge close links with the third (voluntary) sector, as well as with other specialist teams within the Trust and within Primary Care and Partnerships. The role will be based at Mill View Hospital in Hove, and you will be required to carry out home visits as part of the role as well as seeing patients on site.
We are looking for (0.8 WTE) experienced Mental Health Practitioners, from a professional background of Nursing, OT or Social Work who can demonstrate excellent assessment and communication skills, in working with people in the community experiencing a mental health crisis and the ability to deliver a compassionate, person-centered and high quality service to patients, carers and others who are referred to the Neighbourhood Mental Health Team. You will need to have experience of assessing and managing risk in mental health.
