Job description

Are you an aspiring Band 5 clinician looking to make a real difference? We have an opportunity for you to join our vibrant, supportive multi-disciplinary team within the CAMHS Neurodevelopmental Conditions Service (NDC)!

Key Responsibilities
  • Manage a caseload of young people, ensuring high-quality care and support.
  • Develop care pathways and effectively signpost young people to appropriate mental health services.
  • Provide comprehensive assessment and intervention for children and young people experiencing moderate to severe mental illness.
  • Support the duty clinician and respond to urgent cases as required.
  • Offer consultation, advice, training, and support to partnership agencies and colleagues within the multi-disciplinary team.

  • You will be responsible for developing and delivering high standards of assessment and treatment for patients referred to the service, managing an agreed clinical caseload, and ensuring continuity of care through effective care coordination.
  • In collaboration with patients, carers, and clinical teams, you will assess and provide evidence-based treatment programmes and discharge plans, utilising the Care Programme Approach (CPA) to ensure comprehensive patient care. You will also plan and deliver appropriate therapy services, adhering to evidence-based practices and resource constraints.
  • This role involves developing and maintaining relevant, evidence-based therapeutic activities tailored to patient needs, with regular review and evaluation of treatment effectiveness involving patients, carers, and clinical team members.
  • You will actively participate in clinical meetings, case conferences, and CPA meetings, facilitating smooth patient transitions through various care stages, including planning and therapy programs. Additionally, you will compile reports and ensure patient documentation is accurate and up to date.
